ENROLLMENT AND DECLARATION DOCUMENT—HOME DEPOT
Collaborative Concepts LLC (“Collaborative Concepts”) and Home Depot (“Home Depot”) are party to a National Supply Agreement (“National Supply Agreement”), which provides discounts and rebate opportunities for Collaborative Concepts and its Customers (“Customer”);
The business named below, a Collaborative Concepts’ Customer is enrolled in Collaborative Concepts’ National Supply Agreement.
In order to incentivize and reward Customer for its participation, Collaborative Concepts and Customer acknowledge and agree to the following:
A. Contract Discounts:
All National Supply Agreement discounts will accrue directly to Customer
B. Rebates:
The National Supply Agreement offers rebates based on the aggregated spend volume of Collaborative Concepts and its Customers on an annualized basis (calendar year).
Rebates earned by Collaborative Concepts and its Customers will be distributed as follows:
- 2.5% of qualified purchases.
- So as to mitigate administrative burden, rebate checks calculated to be less than $100 will not be distributed.
- Collaborative Concepts anticipates final rebate calculations and checks distributed in approximately March of each year based on prior year spend.
